- Have you traveled to any countries outside of Singapore within the last 14 days?
- Are you or a family member subject to a Stay Home Notice (SHN) or Leave of Absence (LOA)?
- Are you in or are you like to be in contact with infected COVID-19 patients or people in close contact with those patients?
- Do you have a fever (37.5 ℃+), cough, sore throat, body pains, fatigue feelings, difficulty in breathing or any other symptoms of respiratory diseases?
